1. Purpose of CDW

This Collision Damage Waiver (CDW) is offered by J-LINKS FLEET to reduce or waive the financial responsibility of the renter in the event of damage to the rental vehicle during the rental period, subject to the terms below.

2. CDW Terms

By signing below, the renter understands and agrees that:

The CDW is not insurance and is optional.

• If accepted and paid for, the CDW waives the renter’s responsibility for physical damage to the rental vehicle, subject to exclusions listed below.
• A deductible of $___ may still apply for certain types of damage.
• The CDW does not cover:

○          Damage caused while violating the Rental Agreement (e.g., reckless driving, unapproved drivers, DUI, etc.)

○          Tire, rim, glass, or undercarriage damage unless caused by a collision

○          Interior damage, stains, or burns

○          Theft, unless proper police reports and keys are returned

○          Acts of vandalism without a valid police report

○          Any damages exceeding normal wear and tear

3. Responsibility Without CDW

If the renter declines the CDW, they accept full financial liability for:

• All repair costs related to collision or damage
• Loss of use charges during downtime
• Towing and storage costs

4. Rates

5. Renter Acknowledgment

I understand that the CDW is optional and has been offered to me. I have had the opportunity to ask questions and receive clarification.

• I ACCEPT the Collision Damage Waiver at the rate of $___ per day.
